Payroll Title of Employees Directly Supervised:

Primary Purpose of Position: will provide case management services to adults and children with intellectual developmental disabilities and all of mental health and substance abuse groups. You will monitor, treat, and assess the emotional psychiatric and behavioral needs of this special population, will assist in the placement of mentally ill families/ caregivers with mental illness and co-occurring mental illness/substance abuse issues. Staff will provide service coordination activities, oversight, development, and revision of the Person-Centered Plan and monitoring of the implementation of the person centered plan. Qualified professional staff will coordinate, link and monitor the educational, vocational, residential, medical, mental health treatment, financial, social and other non-treatment needs of the individual and family/caregiver.

Work may be performed in a variety of settings including

office, home, schools, institutions, or the community. The ability to adjust to changes with
consumers and within the agency is necessary. This position is under the supervision and guidance of the Executive Director and is subject to a performance review and appraisal at least
once per year. This position is exempt, under federal labor laws and regulations.

Description of Responsibilities and Duties:
Maintain a primary case load of adults with mental illness, intellectual developmental
disabilities, and with co-occurring mental illness and substance abuse issues.
Coordinate initial and on-going assessment activities to include obtaining psychological,
psychiatric, medical, social, family, developmental, substance abuse, educational, legal,
and financial/economic histories
Initial development and on-going revision of the person-centered plan
Development of crisis prevention and intervention plans
Explain and link consumers and families to available services
Supervise paraprofessionals and complete their evaluations
Read documentation notes
Provide information to and consult with consumers and families/legally responsible
persons on person centered plans and other issues
Assess and monitor a consumer's progress and psychiatric/emotional stability
Link consumers to necessary treatment and service needs
Assist consumers and families in obtaining and accessing services
Providing mental health and/or substance abuse education and support to consumers and
families
Provide or arrange for individual and group therapy for consumers, as indicated in person
centered plan
Serve as liaison between consumers and other mental health providers and other
community agencies
Assist in arrangements for consumer transportation for mental health services
Clinically consult with supervisor, treating physician or other service providers to
coordinate treatment
Participate in on-call rotation for consumer base of Carolinas Home Care Agency, Inc.
as needed
Maintain accurate and timely documentation of need for services, services provided and
service outcomes for all assigned clinical cases and services in accordance with North
Carolina DDIMHISAS standards
Completion of necessary assessment tools and documentation to meet agency
requirements (i.e. Person-Centered Plan, NC- TOPPS, IPRS, Internal Authorizations,
admission assessments, client orientation, documentation notes, etc.)
Attend and participate in clinical training, in-service training, workshops and conferences
Attend and participate in scheduled administrative staff meetings
Serve on internal agency committees, such as task forces, QA/QI, and other committees
as appropriate
Provide special consultation in consumer and stakeholder satisfaction studies and other
quality improvement activities
Participate in scheduled clinical and administrative supervision
Maintain and enhance clinical skills through professional readings and attendance at
recommended training events
Other activities, that relate to job title duties
Minimum Education, Training and Experience Required: An individual who holds a license,
certificate, registration issued by the governing board regulating a human service profession OR
Graduate with a master's degree in a related human services field and one (1) year experience
with the population served or graduate with a four (4) year bachelor's degree in a related human
service field and two (2) years' experience with the population served.
