Examples of Duties

  • Facilitate referrals for chronic disease self-management education in both individual and group formats, promoting increased understanding and engagement in long-term health improvement strategies.
  • Assist uninsured participants by linking them to appropriate community-based health and wellness programs to address gaps in care.
  • Identify and refer participants to accessible community locations that provide educational opportunities and internet access to support their ongoing resource needs.
  • Educate individuals on the significance of substance use treatment and connect them with appropriate community-based support services.
  • Provide one-on-one support to help participants access accurate health information and effectively navigate complex healthcare and social service systems.
  • Deliver limited case management services for detainees enrolled in the program, including support in understanding and adhering to their chronic disease treatment plans.
  • Conduct culturally competent health and substance use education in accordance with agency protocols, focusing on chronic disease prevention, physical activity, and nutrition. Maintain accurate documentation of all patient-related activities in compliance with established procedures and protocols.

Minimum Qualifications

High School Diploma or GED and two (2) years of related experience involving performing support services to persons experiencing or at risk of problems related to mental illness and/or substance abuse, or an equivalent combination of education and experience.

Knowledge, Skills And Abilities

  • Knowledge of the social, economic, and environmental conditions of clients served, characteristics of behavior of mental health and substance use, and available community resources
  • Knowledge of community resources
  • Skilled in the use of interpersonal skills
  • Ability to relate to clientele in a supportive and therapeutic manner; to follow written and verbal directions; to record basic data
  • Ability to prepare a variety of reports and documents
  • Ability to effectively use computer applications to collect, organize and present data; ability to communicate effectively in oral and written forms
  • Ability to build and maintain cooperative and effective relationships with clientele, co-workers, and resource staff
  • Ability to work independently and with a team
  • Ability to maintain effective working relationships with officials, management, department heads, and other employees/participants
  • Ability to communicate orally and in writing

Driving is an essential function of this position. To be considered, you must have a valid driver's license with an acceptable driving record. If your driver's license is from another state, you will be required to obtain and show proof of a valid NC driver's license within 60 days of hire.
