Gage House is a 15-bed residential facility licensed to concurrently treat substance use disorders and mental health Illness. It is a voluntary treatment program for men and women aged 18 and older. Gage House offers structured treatment activities, individualized to meet the needs of each person entering the program. Individuals can expect to participate in group therapy multiple times per day with a variety of topics and activities. They will also receive regular contact with a psychiatrist and sessions with a counselor. Several MAT options are available to treat substance use disorders while at Gage House. Program-embedded CRS staff also support individuals in early recovery as part of program standards. Family participation is encouraged in weekly group and individual sessions as needed. The length of treatment depends on the needs of each person; stays vary in length from 21-90 days as determined by the clinical team. Individuals will also be assessed and linked with community providers as deemed appropriate during their treatment stay.

POSITION SUMMARY:

This position is responsible for the treatment and care coordination for clients at Gage House. Duties include: level of care assessments, intake/orientation, developing and implementing preliminary and comprehensive treatment plans, as well as providing individual and group counseling to provide clients with the opportunity to work towards treatment goals and explore recovery. The incumbent will also coordinate aftercare services for services after discharge. The incumbent is responsible for the documentation of these services within the timelines designated by the agency and regulatory bodies.

MINIMUM QUALIFICATIONS:

  1. Bachelors level degree in related field or equivalent education and experience as determined by agency standards and guidelines of position.
  2. Must have valid Pennsylvania licensure or Pennsylvania Certification Board credentials, including LPC, LCSW, LSW, CAAC, CADC, CAADC, or CSS or be able to acquire licensure or credentials within two (2) years of hire.
  3. Exceptional communication skills, both written and verbal, organizational skills and the ability to effectively work with a team.
  4. Strong clinical skills with working knowledge of mental health system, drug and alcohol system and recovery principles.
  5. Ability to effectively work with persons with mental illness and develop comprehensive treatment / recovery plans for their program participation.
  6. Act 33/34/31 and FBI Clearances.
  7. Valid driver's license with access to a vehicle during scheduled work hours.
