Overview
The role of the Social Worker is to create an environment in the home that provides opportunity for the community members to enjoy the highest possible quality of life and provides psychosocial support to residents, families, and partners, which will enhance the quality of life for residents and will promote the health of the intentional community of home.

Shift: Part Time 2p-6p
Responsibilities
- Know each resident and their family including a psychosocial profile of the resident and an understanding of the family members and dynamics.
- Participates in care planning for resident needs.
- Assures that out of facility activities are available to residents.
- Understand and complete MDSs timely and accurately
- Lead the Grievance Committee, hearing any resident concerns and following up on them timely.
- Understanding the ability to complete investigations with the assistance of the Executive Director
- Provide annual education on Abuse, Neglect, and Misappropriation
- Issue NOMNC, and ABN information and allow for appeal process.
- Support and encourage social work interns as needed
- Includes families in the planning process.
- Understands the process for Medicaid Waiver and helps support transitions of care
- Evaluates the effectiveness of the programs and activities.
- Weekly support to each level of care in Independent Living, Assisted Living and Nursing.
- Completes PAS/RR and LOCs on admission
- Documents in accordance with the organization’s policies.
- Complete admission assessments
- Performs any other duties as assigned.
- Coordinates resident discharges.
- Coordinates care conferences.

- Recognize the autonomy and dignity of all residents.
- Communicate a sense of caring, concern, and dignity for residents.
- Understand how to place decision-making in the hands of the residents whenever possible and appropriate.
- Make prompt and accurate judgments with regard to resident care and emergencies.
- Work and communicate effectively as part of the self-directed work team.
- Education: Post-secondary education preferred
- Licensure: OH Licensed Social Worker
- Experience: 2 years related work experience preferably in a nursing home setting
