Family Service Coordinator - Supportive Housing

POSITION OVERVIEW
The Family Service Coordinator (FSC) in Supportive Housing will assist clients experiencing homelessness with housing search and placement activities and provide case management services to reach goals. The FSC will have an involvement in local committees such as Housing Support Meeting, HIT, the Housing Coalition, and Heading Home Committee.

AREAS OF RESPONSIBILITY AND ACCOUNTABILITY
- Maintaining composure and working effectively under stressful conditions.
- Develop and write client plans specific to emotional, physical, social and cultural issues in an effort to reduce housing barriers.
- Work directly with an integrated team of staff from CAP, the County, and other organizations to determine referred program eligibility.
- Develops housing resources by establishing relationships with landlords and short-term stay residences in order to link clients to them and other local resources.
THE IDEAL CANDIDATE
The ideal candidate will bring knowledge of the principles of Trauma Informed Care, Person Centered Theory, and Housing First. They will have experience working with mental health and chemical dependency populations. They should have the principles of record keeping and record management as well as the ability to quickly adhere to our housing policies, procedures and applicable CAP software that we utilize and a passion for serving people that were formerly or are currently unhoused.
ADDITIONAL QUALIFICATIONS
A Four-year degree in social work or related field plus one year of associated experience or equivalent combination of education and/or experience. Experience working with individuals or families who are unhoused and/or from various social backgrounds. Special consideration given to bilingual candidates.
