POSITION SUMMARY:
The On-Campus Case Manager will work directly with Eric Birch High School - Fontana Unified School District. They will use their clinical expertise to understand and address the needs of both students and their families. This role involves providing individualized case management by connecting families to community resources and ensuring those services are accessed and utilized effectively. The Case Manager will also lead group workshops focused on psychoeducation, including parenting support and the impact of mental health challenges in school settings. In addition, they will offer one-on-one support to students and caregivers, help strengthen family engagement, and serve as a key link between school staff and families to promote student well-being and success.
Requirements:GENERAL REQUIREMENTS:
Education/Experience
- Master’s degree in Social Work, Counseling, Psychology, or related field required.
Licensure, Registration, Certification
- PsyD/PhD/LCSW/LMFT required.
- Valid CA driver’s license and proof of automobile insurance.
- Completion of CPR/First Aid within the first thirty (30) days of employment.
JOB DUTIES & RESPONSIBILITIES:
- Provide comprehensive case management by linking students and families to appropriate services and community resources
- Conduct follow-ups to ensure successful linkage, including appointment scheduling and resource utilization
- Facilitate psychoeducational group workshops for students and caregivers, including parenting groups and mental health education
- Deliver individualized parenting support through one-on-one check-ins and engagement strategies
- Act as a liaison between school staff and caregivers to strengthen family involvement in student success
- Offer individual support to students through regular check-ins
- Identify and address student needs in collaboration with school personnel and family systems
- Complete documentation and other administrative tasks within required timeframes.
- Performs other related duties as required and assigned.
PHYSICAL REQUIREMENTS:
- Ability to drive a motor vehicle without restrictions, often in heavy traffic and frequently long distances (one hour or more) to ensure safe transport to client settings and to safely transport clients to appointments as needed.
- Senses of vision, hearing and smell sufficient to regularly operate a motor vehicle, to visualize threats in the environment or risks to clients, and to operate safely in the field.
- Required to occasionally lift and carry 10-20 pounds.
- Regularly required to sit, stand, climb stairs (1 or more flights) and walk while in route to and during family visits.
- Regularly required to sit and use computer in office settings.
