Summary
Neighborhood Worker ensures social service and medical assistance work are provided to individuals and families in a neighborhood community program in a county area. Employees in this classification provide services of a non-professional nature to families and individuals relating to social service, general health or mental health care, depending upon the area of assignment. Emphasis of the work is on assisting individuals and their family members in designated neighborhood areas in obtaining appropriate social or legal service, general health or mental health care by interviewing clients, helping them to plan, and contacting community agencies.

Responsibilities
- Contacts area residents in their homes, or as they call at office.

- Assists clients in obtaining needed assistance, such as emergency food, social security benefits, legal services, unemployment benefits, medical attention and related services.

- Interviews clients in a designated area seeking assistance in a program of community mental health services.
- Visits homes in the neighborhood to answer questions.

- Arranges client transportation to medical units.
- Explains details of admission and release from medical care.
- Initiates and keeps client files up-to-date by recording medical visits and follow-up; keeps contact with clients as they are hospitalized.
- Aids in developing programs to motivate clients to avail themselves of social or health services.
- Maintains contact with community resource agencies to keep informed on methods of referral for clients.
- Participates in meetings with other staff members to discuss needs of clients and their families.
- Attends various in-service training sessions.
- Demonstrates behaviors of service excellence and CARE values (Compassion, Accountability, Respect and Expertise).
- Performs all other related job duties as assigned.
- Generally requires 0 to 3 years of related experience.
- Knowledge of HIV/AIDS disease & services preferred.
- Bilingual Spanish or Creole preferred.
High School diploma is required.
