The Community Liaison is responsible for the distribution and receipt of signed physician orders for the agency as well as:

Essential Responsibilities and Duties: (Note: Other Duties May Be Assigned)

• Initiate and maintain healthcare contacts within the assigned territory.
• Maintain contact with hospitals, physicians, physician’s staff, discharge planners, social workers.
• Serve as a liaison between patients, physicians and agency staff to ensure that the patient’s needs are being met.
• Pre-screen patients from physicians, hospitals, long-term care facilities and assisted living facilities for home health needs, eligibility and status, when referral is concurrent with discharge.
• Make marketing presentations to current and prospective clients to educate them on the agency’s services.
• Deliver orders to physician’s offices for signature and ensure timely return of the orders.
• Communicate with each referral source to ensure that the expectations of the referral sources and the needs of the patient are being met.
• Provide timely, accurate completion of reports, worksheets, and information as requested by the Administrator/designee.
• Participate in company-sponsored programs and meetings and maintain a networking base in the community through trade associations, business organizations and local association meetings.
• Maintain knowledge of the agency’s policies and procedures and Medicare regulations that are applicable to home health care.
• Able to work cooperatively as a member of a team.

Education and/or Experience:

• 2+ years of sales experience in home care and/or health-related field preferred
