The Care Management Assistant supports CDNY Care Management services by the model of care and regulatory requirements of the applicable program or line of business.
The Care Management Assistant provides support to care management teams by completing non-billable tasks and duties assigned by their designated regional supervisor. The Care Management Assistant performs functions that assist Care Managers in coordinating and implementing the person-centered planning process.

- Assisting in obtaining documents needed for person-centered planning and core Care Management responsibilities (i.e., medical records, Staff Action plans, documents required for waiver applications)
- Contacting providers to assist Care Managers with service linkages/referrals (i.e., calling providers for waiver service openings; helping facilitate the completion of referral packets)
- Attend regional rounds consistently Present at regional rounds to assist Care Management with the most complex
- Assisting Regional Care Management Teams in achieving performance metrics through regional tracking systems.
- Assist with coordinating scheduling meetings with the assigned Care Manager, Member/Family, and members of the IDT.
- Assist in gathering necessary documents that ensure a complete and comprehensive record. Engage with members/family, service providers, and members of the IDT to obtain and upload documents into the member’s record for the assigned Care Manager’s review and action.
- Participate in the CAS assessment with the assigned Care Manager as
- Attend the Care Manager review of CAS assessments as
- Attend LP meetings with the Care
- Review documentation and upload it to preventative care with the director’s
- Participate in the completion of DDP2s with the assigned Care Manager.
- Prepare biopsychosocial assessments as
- Ensure that all assigned duties are completed on
- Ensure confidentiality and adhere to HIPAA
- Navigate Microsoft Office applications, including Outlook, Word, Excel, and

Qualifications & Experience
- High School Diploma
- A completed or in-process bachelor’s degree in human services is
- Personal or professional experience working with individuals with I/DD
