Community Dental Health Care Coordinator (CDHC)
The mission of WCDC is to provide underserved residents of Central Maine with quality dental care to achieve good oral health and overall well-being. To reach this goal, patients and families need support to navigate complex systems, access available resources, and remove barriers to care. This is the role of the CDHC.
More specifically, the CDHC will:
· Work with the community-based Maine Dental Connection team to coordinate the delivery of community-based teledentistry care in settings that may include preschools, schools, and residential facilities, including facilities for seniors. · Coordinate care for children who need hospital-based treatments with sedation in the operating room.
· Assist in the development and coordination of activities to integrate medical and dental programs, including integration of dental services with primary care and prenatal programs.
· Assist patients who need extra support with navigation of their care, including coordination of appointments, transportation, and assisting with any other barriers to successful completion of treatment.
Key responsibilities include:
- Assist as needed with patient care activities to include scheduling, confirmation phone calls, retrieving/dispensing electronic charts, data input, arranging and following up on referrals, referral for resources including transportation that may be needed for the successful completion of evaluation and treatment plans.
- Coordinate activities of the WCDC based Maine Dental Connection (MDC) program that provide community-based dental services with the support of Teledentistry:
- Develop relationships with educational, social, and general health systems such as Head Start preschools, schools, and other settings which can host onsite dental services
- Assist with explaining the MDC policies/procedures as related to dental treatment and the evidence base supporting community-based care to patients, parents, caregivers, and agency staff
- Collaborate with community site staff and the dental clinical team to create systems to obtain patient consent, develop workflows to provide on-site services, and support the provision of those services
- Support the collaboration of the dental team, including the onsite team providing the on-site care and the dentist who will determine a treatment plan for each patient in the program
- Assist clients with enrollment in oral health care coverage or insurance.
- Assist with collecting and recording electronic data
- Provide oral health education as directed by the clinical staff
- Attend and participate in staff meetings and other staff events
- Run clinical and data reports as needed to fulfill reporting obligations and maintain appropriate program surveillance. Meet periodically with personnel from site and additional evaluators.
- Maintain a relationship with and assist with applications and reports to supporters and funders of the program, including the United Way
- Develop and maintain current knowledge of oral health as well as different types of dental benefits and insurance. WCDC will assist in training as needed.
Qualifications:
Education:
· High School Diploma or equivalent
· College diploma with knowledge of Care Coordination and/or Public Health principles and practice preferred but strong experience base and/or ability to learn would be considered

Job Types: Full-time, Part-time
Pay: From $21.00 per hour
Expected hours: 32 – 40 per week
