Moorings Park is looking for a Discharge Planner. The Discharge Planner coordinates and facilitates resident discharge planning under the direction of and in collaboration with the registered nurse, physician, members of the therapy team, and Interdisciplinary team.
Contributions:
- Provides a friendly, pleasant welcome for all who contact and visit the offices.
- Provides tours to potential residents and their families.
- Verifies insurance information, demographics, and emergency contacts provided.
- Reviews the admission contract with the residents or families and obtains signatures for admission documents.
- Collaborates with the registered nurse, physicians, caregivers, residents, family, other departmental team members, and the payor to proactively develop and implement a safe and appropriate discharge plan.
- Assists as needed with obtaining referrals, prior authorization for Home Health Care, DME, SNF, acute rehabilitation, and appointments.
- Maintains working knowledge of Medicare, and private insurance company coverage for referred products and services.
- Participates in and completes discharge assessments, completes follow-up phone calls promptly, and provides referrals/data according to the patient's needs.
- Documents and tracks specific needs for GG compliance.
- Maintains a current list of resources for referrals and refers to the appropriate inpatient, outpatient, and community resources.
- Provides accurate census and room management to staff of The Chateau.
- Serves as backup for Admissions Coordinator
- Provides Manager on Duty Services on a rotation.
- Certifications and licenses required by state regulations.
- Works with confidential data. Actively promotes patients' confidentiality and rights. Disclosure would be contrary to the best interest of the facility and professional ethics.
- Excellent oral and written communication skills in the English language required
- During emergencies or natural disasters may be required to be on duty for the duration of the disaster.
- A degree in a relevant field, such as nursing, psychology or social work.
- One year's experience working in a long-term facility.
- Certifications and licenses required by state regulations.
