Conducting nursing assessments with prospective and current clients, addressing their questions and concerns.
Providing active care management for our medication clients. Active care management includes regular home visits, management of our care plans, reassessments, and support for our clients’ needs. Active Care Management will be at minimum aligned with the standards set forth by the Department of Health within your designated State.
Ensuring that you meet or exceeds regulatory requirements with your designated State.
Responding to client or caregiver reported incidents in the home, complaints, or quality of care

concerns and ensuring that all parties are satisfied with the ongoing care plan. This may involve
updates to the care plan or in-home training of the caregivers as needed. * Ensuring that our caregivers meet the requirements set forth within your State, are adequatelytrained to deliver care, and receive RN instruction as needed. This includes leading the practical
skills portion of any trainings and re-trainings. * Maintaining all client records, notes and files and ensuring that activity is properly documentedRequired Skills, Education and Certifications:

Current Registered Nurse license in the state of employment; continued employment is contingent upon maintaining active license in good standing
Three to 5 years’ experience in health care, elder care, social work or related industry
Excellent customer service and conflict resolution skills
Computer proficiency and ability to document timely and accurately notes in system
Current driver’s license and proof of insurance

Physical Requirements:

Ability to travel to client homes and other locations 50 – 75% of the time
Ability to lift and carry up to 10-15 pounds
Ability to sit, stand and walk for prolonged periods of time throughout the work day
Ability to use standard office equipment such as personal computer, laptop, telephone, photocopy machine, etc.
