The Wisconsin Veterans Home at Chippewa Falls provides compassionate and professional long-term, skilled nursing care to veterans and spouses who cannot be cared for in their homes.

As a Care Coordinator at the Veterans Home you will:

• Serve as the primary point of contact for residents regarding their appointments.
• Coordinate resident referrals and follow-up appointments with specialists.
• Maintain accurate patient records and ensure all documentation is up-to-date.
• Assist in managing resident inquiries and resolving any issues related to their care.
• Ensure compliance with healthcare regulations and maintain confidentiality of patient information.

As the Care Coordinator you will possess:

• Experience providing medical administrative support will be highly regarded.
• Strong organizational skills and the ability to manage multiple tasks simultaneously are critical for success in this role.
• Excellent interpersonal skills to interact effectively with patients, families, and healthcare professionals.
