Overview:
The Discharge Planner is an integral part of the interdisciplinary team, and also provides community outreach, networking, and liaison with community programs as needed.
The Discharge Planner provides services to patients, families, and others in all hospital areas specific to assignment as applicable, and in accordance with hospital customer service expectations. Professional services to be provided to meet the discharge planning needs of the patient may include, but are not limited to, case consultation, patient education, resource management, and assistance with discharge planning.

- This position requires the full understanding and active participation in fulfilling the mission of AHMC- Anaheim Regional Medical Center and AHMC Inc. It is expected that the employee demonstrate behavior consistent with the core values of AHMC- ARMC and AHMC Inc. The employee shall support AHMC- Anaheim Regional Medical Center’s strategic plan and goals and direction of the performance improvement plan. The employee will also be expected to support all organizational expectations including, but not limited to; Customer Service, Patient’s Rights, Confidentiality of Information, Environment of Care and AHMC Inc. initiatives.
Education/Training/Experience
Bachelor’s degree from an accredited school of Social Work or related field,
RN or LVN preferred
Two years acute or related medical care experience.
