Our client, a federally qualified healthcare center, is seeking to hire a Licensed Clinical Social Worker, LCSW.


  • Location: Queens, NY. Onsite.
  • Schedule: Full Time: Daytime Hours.
  • Compensation: The salary range is up to $90K depending on years of experience. Our client provides an exceptional benefits package.

Opportunity Description:


The position is responsible for performing Mental Health for patients to ascertain their diagnoses. Providing care to a diverse and underserved population in an outpatient setting.

Job Duties:

  • Performs Mental Health Assessments for patients to ascertain their diagnoses and establish treatment plans Provides individual and group counseling and psychotherapy
  • Provides crisis intervention and/or counseling to diffuse conflicts among patients
  • Completes medical forms and documentation related to patient care including any necessary to facilitate transfer, and if indicated, referrals to outside programs or psychiatric clinics
  • Makes and/or tracks all referrals to other mental health agencies and providers
  • Works collaboratively with specialists for consultation and referral purposes
  • Works with others on the intake team to help determine whether or not a patient would be best helped by the facility or another agency.
  • Meets with other clinical staff to review cases
  • Maintains confidential case files, including documenting requests for confidential information
  • Follows the clinical chart review policies and procedures
  • Works in a team environment
  • Provides workshops for families on communication and other family-related issues
  • Coordinates emergency responses, including a team to minimize patient's danger to self and others, direct discussion with EMS/FDNY regarding patient's condition, referral to psychiatric or medical emergency departments and follow-up while patient is in the emergency department or upon release
  • Maintains ultimate responsibility for directing the course of the patient's mental health care and treatment plan within the scope of his/her expertise
  • Encourages health promotion, disease prevention, and health maintenance
  • Provides patient advocacy, as appropriate for the site
  • Participates in case-conference and case management activities


Education:


  • Master of Social Work (MSW) degree from an accredited institution.


Experience & Skills Required

  • Minimum of one clinical experience preferable.
  • Familiarity with word processing and other computer applications.
  • Demographic, Registration, Appointment/Scheduling, Scanning, Triage, PMH/Vitals, Orders, Immunizations/PPD, Progress Note, Reporting, Labs, Medical Summary, Telephone Encounters.


Certifications:

  • Unrestricted New York State Clinical Social Worker license.
