Northwoods Rehabilitation & Nursing Center is seeking a caring and organized Social Worker / Scheduler with a passion for working with our residents, to fill a full-time position at our skilled nursing facility. In this dual role, you will support residents and families while also scheduling medical appointments and coordinating transportation needs.Responsibilities:Provide social support and guidance to residents and their familiesAssist with care planning and discharge arrangementsSchedule resident medical appointmentsCoordinate transportation for off-site appointmentsMaintain accurate records and clear communication with staff and providersRequirements:Experience in social work or a healthcare setting preferredStrong organizational and communication skillsAbility to multitask and manage time effectivelyExperience with scheduling and transportation coordination a plusApply today and make a difference in the lives of others!
