Job Title: Registered Nurse Case Manager (RNCM)
Location: Winnebago County, IL (Hybrid: Virtual & In-person)

About the Role:
The RNCM will manage a panel of members with chronic disease and behavioral health needs, creating and overseeing care plans, collaborating with healthcare providers, and coordinating resources to ensure comprehensive care.

Responsibilities:

• Conduct assessments, including behavioral health screenings (e.g., PHQ-9, GAD-7).
• Develop and monitor care plans with members, addressing social and medical needs.
• Participate in case conferences, provide clinical follow-ups, and support chronic disease management.
• Administer medication reconciliation and ensure patient compliance.
• Use care facilitation tools and electronic health records for documentation and communication.

Qualifications:

• 3+ years of nursing experience.
• Graduate of an accredited nursing school (R.N.).

This hybrid position combines virtual care and in-person visits in Winnebago County, Illinois.

Pay: $90,000.00 - $110,000.00 per year
