Job Summary

As our field-based RN, you’ll be the clinical anchor across multiple Twin Cities locations. You’ll manage the health needs of residents in supportive apartment settings—delivering person-centered care, preventing crises, and collaborating with a multi-disciplinary team to support individuals navigating mental health conditions, developmental disabilities, and physical health challenges.

What You’ll Do

  • Monitor and assess residents’ physical and mental health
  • Administer or set up prescribed medications
  • Record and chart vital signs, observations, and progress notes in the EHR
  • Report changes in condition and act promptly
  • Collaborate with support staff and healthcare professionals to plan and modify care
  • Assist with appointment coordination and documentation
  • Provide preventative care and crisis de-escalation
  • Ensure quality care through strong communication and compassionate presence
  • Other clinical and supportive duties as assigned

Who You Are

  • Licensed RN in the state of Minnesota
  • Experienced or interested in working with individuals with disabilities or mental health needs preferred but not required
  • Tech-savvy and comfortable with electronic health records
  • Calm under pressure, with great documentation and people skills
  • Able to work independently, travel to multiple sites, and be a resource to others
  • Driver’s license + reliable, insured vehicle required

Bonus Points For

  • Previous home health, long-term care, or public health experience
  • Mental health experience

Schedule + Work Style

Expect flexibility! This role is essentially Monday through Friday, but may require on-call telephone responses outside of normal business hours as needed by the program.
