Job Summary
As our field-based RN, you’ll be the clinical anchor across multiple Twin Cities locations. You’ll manage the health needs of residents in supportive apartment settings—delivering person-centered care, preventing crises, and collaborating with a multi-disciplinary team to support individuals navigating mental health conditions, developmental disabilities, and physical health challenges.
What You’ll Do
- Monitor and assess residents’ physical and mental health
- Administer or set up prescribed medications
- Record and chart vital signs, observations, and progress notes in the EHR
- Report changes in condition and act promptly
- Collaborate with support staff and healthcare professionals to plan and modify care
- Assist with appointment coordination and documentation
- Provide preventative care and crisis de-escalation
- Ensure quality care through strong communication and compassionate presence
- Other clinical and supportive duties as assigned
Who You Are
- Licensed RN in the state of Minnesota
- Experienced or interested in working with individuals with disabilities or mental health needs preferred but not required
- Tech-savvy and comfortable with electronic health records
- Calm under pressure, with great documentation and people skills
- Able to work independently, travel to multiple sites, and be a resource to others
- Driver’s license + reliable, insured vehicle required
Bonus Points For
- Previous home health, long-term care, or public health experience
- Mental health experience
Schedule + Work Style
Expect flexibility! This role is essentially Monday through Friday, but may require on-call telephone responses outside of normal business hours as needed by the program.
