About Job

For students that are actively enrolled in a Social Work program We are currently accepting applications for an Social Work internship at our Adult Day Care. The intern is responsible to monitor mental health status and provide case management services to program participants under the supervision of Program Coordinator and ADH Director. We are open 8:00 a.m. - 4:00 p.m. Monday through Friday.Duties and Responsibilities:
  • Provide case management services
  • Develop and lead groups for the participant.
  • Assist the program coordinator in referral, admission and discharge process.
  • Write progress notes quarterly.
  • Inform participants and their families/caregivers of available community services, and make referrals as indications warrant.
  • Attend weekly 1:1 supervision with ADH Director/internship supervisor.
  • Assist, as necessary, in the delivery of other required program services.
  • Participate in the bi-annual or quarterly review of each participant’s care plan as well as assisting with the development of care plans.
  • Assist Program Coordinator with the implementation of policies and procedures.
  • Assist Program Coordinator with record reviews, documentation, compliance, and implementation of policy and procedure.
Education and Experience:
  • Must be actively enrolled is BSW program Bachelors Degree required.
  • One (1) year of recent experience working with disabled/elderly adults in a professional capacity is required.
