- Answer patient questions and provide information about the clinic's services, procedures, and policies.
- Assist patients with scheduling appointments and follow-up visits.
- Address patient concerns and provide support to ensure a positive experience
- Coordinate and order medications for patients as prescribed by healthcare providers.
- Ensure timely and accurate processing of medication orders and refills.
- Communicate with pharmacies to get accurate pricing and resolve any issues related to medication availability.
- Educate patients on prescribed medications, including usage instructions, potential side effects, and precautions.
- Provide information and guidance on recommended supplements, including benefits and proper usage.
- Develop and distribute educational materials to enhance patient understanding of their treatment plans.
- Maintain accurate and up-to-date patient records in the clinic’s electronic health record (EHR) system.
- Assist with the coordination of patient care, including referrals to specialists and coordination of diagnostic tests.
- Support the clinic’s administrative functions, including answering phone calls, managing correspondence, and assisting with front desk operations as needed.
- Collaborate with healthcare providers, nurses, and other clinic staff to ensure a holistic and integrated approach to patient care.
- Participate in team meetings and case conferences to discuss patient progress and care plans.
- Facilitate communication between patients and healthcare providers to ensure clarity and continuity of care.
- Excellent communication and interpersonal skills, with the ability to interact effectively with patients, families, and healthcare providers.
- Strong organizational skills and attention to detail, with the ability to manage multiple tasks and priorities.
- Proficiency in using electronic health record (EHR) systems and other healthcare software.
- Ability to maintain confidentiality and handle sensitive information with professionalism and discretion.
- Empathy and compassion for patients, with a commitment to providing high-quality patient care.
- Registered Nurse (RN)
- Previous experience in a healthcare setting, particularly in patient care coordination or a similar role, is highly desirable.
- Knowledge of medical terminology and understanding of integrated healthcare principles.
