About the Job: Under the supervision of the Director of Clinical Services, oversees the day-to-day operations and operational effectiveness of the Home Care department to ensure high quality, safe, effective care is provided in conformance with policies, procedures and standards and all governing state and federal regulations. Participates in the Interdisciplinary Team (IDT) Meeting as required, acting as the Home Care representative.

- Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
- Must have a valid driver’s license, proof of insurance and have means of transportation.
- Education: Graduate of a school of professional nursing required. Current Practical Nurses License (L.P.N.), (CPR) - certification required.
- Experience: A minimum of one year’s experience in working with the frail or elderly population required.
Skills/Knowledge:
- Broad knowledge base of health care problems of the frail elderly.
- Knowledge of medical equipment and instruments.
- Knowledge of common safety hazards and precautions to establish a safe work environment.
- Skilled in identifying problems and recommending solutions.
- Skilled in preparing and maintaining records and writing reports, using an EMR and other healthcare software and responding to correspondence.
- Skilled in developing and maintaining department quality assurance.
- Knowledge of medications used in geriatrics effectively in emergency situations. adverse reactions.

Skilled in establishing and maintaining effective working relationships with participants, staff members and family caregivers.
Ability to react calmly and effectively in emergency situations.
- Working knowledge of local health care and geriatric service networks
**Preferred: Experience in the delivery of in-home/home care services and/or home health care.**
ESSENTIAL FUNCTIONS:
- Directly responsible for the operational oversight of all Home Care and Personal Care staff, including Home Care aides (CNAs, PCAs) including schedule management, and overall operational effectiveness of the department.
- In coordination with the Director of Clinical Services, performs orientation, initial and annual competencies, and is responsible for evaluating performance of all home care staff.
- In coordination with the Director of Clinical Services, performs quarterly supervisory visits in the home of each participant receiving home care services.
- Prioritizes utilization of home care staffing needs to reflect actual enrolled participant’s home care needs.
- Performs initial, semi-annual & change of status assessments of participants in their home environment to determine their individual needs and coordinates plan of care with caregiving resources.
- Coordinates with the Interdisciplinary Team to develop plan of care for each participant.
- Ensures home care staff follow the participant’s plan of care as approved by IDT.
- Functions as a member of the Interdisciplinary Team. Maintains regular attendance at, and participates in Interdisciplinary Team meetings, communicates participant changes, collaborates on plan of care decisions and coordination of twenty-four (24) hour care delivery.
- Maintains participants’ medical record and fulfills charting and reporting requirements per policy.
- In coordination with the Director of Clinical Services, monitors participant medication adherence, reporting issues or concerns to appropriate clinical and medication department staff.
- Responsible for maintaining compliance with all State and Federal program requirements, maintaining compliance under CMS as well as any other state or federal law or rules for in home services
- Implements quality assurance/improvement systems, policies and procedures per established agency wide protocols and standards. Regularly monitors quality of care and service delivered to ensure compliance with established standards.
- Establishes and maintains a productive working relationship with relevant contracted providers and agency staff.
- Engages in team building. Coaches and mentors staff. Instructs and teaches staff as needed. Trains clinical staff to function independently. Ensures that the job responsibilities, authorities

- Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.

1. Physical Requirements – Requires full range of body motion including handling and lifting patients. Requires standing, walking, pushing, bending, kneeling and reaching at arm’s length and overhead in clinic, rehab department and in community for prolonged periods of time. Must be able to transfer participants needing maximum assist of one. Requires the ability to lift/carry up to fifty (50) pounds and push or pull objects exceeding one hundred fifty (150) pounds, using appropriate body mechanics. Requires manual and finger dexterity and eye-hand coordination.
