The Case Management Coordinator collaborates with patients, families, physicians, and the interdisciplinary team to ensure efficient and cost-effective patient care throughout the care continuum.
- Responsibilities include assessing, planning, implementing, and evaluating individual patient needs, managing patient flow, discharge planning, and supporting safe transitions.
- They handle payer inquiries when appropriate, align with organizational goals, and uphold quality and regulatory standards.
- Requirements include a graduate degree in nursing (BSN preferred) or a Master’s in Social Work (MSW) with appropriate licensure as a Registered Nurse or licensed social worker in New Mexico.
- Certified case management credentials are highly desirable.
- Skills needed are effective communication, teamwork, critical thinking, resource utilization, flexibility, multitasking, and strong organizational abilities.
- Preferred experience is two years in healthcare, with one year in acute care.
- The role involves hospital mobility, office work, and adherence to safety protocols.
