Provides patient care under supervision, following physician-approved protocols to improve health outcomes and reduce hospital readmissions.
- Manages a caseload of 10,000-15,000 patients, identifying care gaps and coordinating preventative and follow-up services.
- Collaborates with the care team, including Nurse Navigators, to facilitate transitions of care and ensure medical stability.
- Reviews medical records, schedules services, and documents encounters using evidence-based guidelines.
- Promotes patient engagement through education and behavioral strategies, fostering team communication.
- Maintains professional licensing, confidentiality, and documentation standards.
- Requires vocational/technical training, LVN/LPN license in TX, basic computer and EMR skills, and at least 3 years of clinical experience, preferably in primary care.
