Healthcare Campus is currently seeking a Full-Time Social Worker with Behavioral Health Experience to join our team !

QUALIFICATIONS:

1. Graduate of an accredited school in social work or related field; having a Masters degree in social work or a Master's degree in a human service field including but not limited to sociology, special education, rehabilitation counseling, and psychology.

2. State certified by the Board of Social Work Examiners as CSW or LSW.

3. Experience in Behavioral or Long Term Care.

POSITION SUMMARY: Performs social services based on the facility’s policies and procedures as regulated by the State and Federal laws. PERFORMANCE EVALUATION: A competency based performance evaluation has been made part of this job description.

JOB RESPONSIBILITIES

• Accurately conducts social service assessments and interventions and documents accordingly within the facility’s standards based on the State, Federal regulations & Joint Commission standards.
• Admissions and Intake
• Care Plan Meetings
• Complete documentation

• Initial Social Service History within 14 days of admission.
• MDS 3.0, Completes CAA’s
• Completes IDCP on admission.
• Updates care plan as needed.
• Annual progress notes, readmissions from hospital and significant change.
• Hospital updates are completed within 48 hours.
• Discharge planning.
• Facilitating resident/family groups.
• Contacting families on various issues.
• Assist with resident satisfaction surveys
• Demonstrates knowledge of Federal regulations.
• Demonstrates knowledge of DOH regulations.
• Demonstrates knowledge of OBRA regulations.
• Participates in interdisciplinary meetings, weekly and as needed.

Job Type: Full-time
