Position Summary
The Certified Nursing Assistant (CNA) focuses on supporting individual's progress towards goals that are identified as meaningful by individuals and their support resources, as well as providing support for the overall health and safety of all the individuals. As part of the Nursing team, the CNA will provide direct care to individuals with multiple ID/DD and chronic medical conditions in the Intermediate Care Facility home-like environment.
Required Education and Experience
- High School Diploma/GED
- Prior direct care experience is preferred.
- Certified Nursing Assistant (CNA)
Related Knowledge, Skills and Abilities
Knowledge of electronic health records and documentation standards as required by the program.
Safely assist individuals with all aspects of physical management including equipment use, safe lifting and physical assistance.
Knowledge and ability to operate a computer, utilize an electronic health record, electronic time sheet, and other software/systems used by Horizon.
Knowledge and ability to utilize personal protective equipment in a manner that is compliant with standard precautions and safety expectations.
Ability to drive and must possess a valid driver’s license. May be called upon to travel to any Horizon worksite; transport clients, provide services to client @other locations. Regular attendance at trainings and Horizon meetings may require travel to a variety of Horizon sites.
Willing and available to work modified schedules (weekends, holiday, etc.) as required.
Ability to participate in and successfully pass behavioral intervention training; includes both verbal de-escalation skills and physical skills techniques to be utilized with consumers as needed.
