Growing home health and hospice agency in Katy, TX is looking for an exceptional Community Liaison to help build relationships and facilitate client referrals. In this role you will:

• Conduct outreach to healthcare providers, social workers, and community organizations to educate on our services to promote referrals
• Manage an outreach territory

The ideal candidate has:

• 2+ year(s) experience in healthcare outreach, admissions, hospice sales and/or business development
• Existing relationships with social workers, discharge planners, case managers, and other referral sources
• Strong communication and presentation abilities
• Motivation to succeed in a performance-based compensation role and exceed goals

Compensation plus commission based on billable hours of services.
