A facility located west of Peoria, IL is seeking a Hospice Social Worker to join their team!

Pay: $26.00 - $39.00 / hourly (Depending on years of experience)

Hospice Social Worker Opportunity:

• Full-time, direct-hire opportunity
• Schedule: Monday - Friday, days
• Hospice social worker role

Hospice Social Worker Responsibilities:

• Under general supervision of the Leader of Home Health / Hospice and Home Health Clinical Care Coordinator and in accordance with departmental policies and professional standards, prepares social histories, provides social casework to patients and families having difficulty in social functioning primarily as it affects their health, refers and aids in the referral process to other agencies, supervises social work assistant, and generally serves as a social work consultant in the HHC department's total program

Hospice Social Worker Requirements:

• Masters in Social Work
• Hold a valid State of Illinois Social Worker License (LSW or LCSW)
• Open to new graduates
