Position Summary: Under the general supervision of the Program Manager and the Clinical Director, the IHOC is responsible to provide comprehensive counseling services to strengthen and preserve families to assure the safety and well-being of children, empower families, and involve the community in family support.
Duties and Responsibilities: The In-Home Outreach Counselor assumes the following duties and responsibilities:
Provide in-home outreach counseling to provide support and educate families on parenting skills, positive discipline, laws regarding child abuse and neglect, goal setting, and family organization;Provide supplemental services: child focused activities; child follow-up visits; parenting training; teaching and demonstrating homemaking skills; counseling on substance abuse, domestic violence or anger management, and career need issues; substitute adult role model; transportation; and emergency housing; Develop and utilize community resources to provide linkage services for drug testing, substance abuse assessment and treatment services, etc.
Complete intakes and assessment forms and maintain up-to-date FAF primary web-based Electronic Case Records system and hard copies case records and back up documents on each client and family served, and other reports in a timely manner; Be available to provide emergency services as needed; Attend MCPC and all other meetings and trainings required by the program; Strictly adhere to confidentiality, privacy and Cyber security policy and protocol; Performed other related duties as assigned.
