The RN Care Manager is responsible for assessing, planning, and coordinating the care of patients from admission through discharge. This includes managing transitions of care, reducing unnecessary hospital stays, and ensuring appropriate discharge planning for high-risk patients. The RN Care Manager works closely with the interdisciplinary care team to promote quality outcomes and efficient resource use.
Key Responsibilities:- Complete comprehensive assessments for all patients on admission.
- Screen for high-risk indicators and initiate appropriate discharge planning early.
- Provide ongoing reassessment and care coordination throughout hospitalization.
- Handle utilization review (UR) activities per policy (eg, clinical reviews, payer reviews, audits).
- Lead transitional planning (eg, arranging aftercare, caregiver education, root cause analysis for frequent utilizers).
- Communicate with patients, caregivers, and healthcare team members.
- Ensure compliance with department goals for length of stay (LOS), readmission prevention , payer denials , and patient satisfaction .
- Document per department standards.
- Collaborate across departments to support discharge needs, especially for geriatric and complex patients .
- Participate in quality improvement initiatives as needed.
- NYS Registered Nurse (RN) license - Required
- PRI Certification - Must be obtained within 3 months of hire
- Screen certification - Strongly preferred
- Care Management certification (CMC, CCM, etc.) - Preferred
- 3 years of clinical experience in ED, Critical Care, or Med/Surg - Preferred
- Prior Case Management experience - Preferred
- Familiarity with healthcare regulations, payers, and discharge planning programs
- Experience using Milliman or InterQual criteria
- Strong computer and documentation skills essential
The RN Care Manager is a registered nurse who helps plan a patient's entire hospital stay-from the moment they're admitted to the moment they go home or to another facility. Their main goal is to make sure patients get the care they need efficiently and are discharged safely with proper follow-up plans. They look out for patients at high risk of complications or readmission , and help coordinate services like home care, rehab, or follow-up appointments. They also work with insurance companies , do clinical reviews , and ensure the hospital follows all care guidelines.
This role is a mix of clinical knowledge , communication skills , and administrative work -ideal for nurses who enjoy both patient interaction and strategic coordination.
