Job Title: First House Healing & Housing Case Manager

Travel: Occasional travel required for client support, outreach, and partner agency collaboration.
Hours: Full Time – 35 Hrs/Wk (Monday–Friday), with flexibility for occasional evenings/weekends based on client needs.
Minimum Salary: $19.75/hr
Maximum Salary: $21.00/hr
Position Summary
The Case Manager provides trauma-informed, individualized case management, housing navigation, and aftercare services for women and children in YWCA Jamestown’s First House transitional housing facility and survivors participating in the Rapid Rehousing program. This new position plays a critical role in helping survivors of domestic violence and other trauma obtain and maintain safe, permanent housing, offering support from intake through post-exit follow-up.
Essential Responsibilities:
First House Case Management:
- Conduct comprehensive intake assessments and develop individualized service and safety plans for residents of First House.
- Provide regular case management focused on goal setting, safety, employment, education, parenting, health, and personal empowerment.
- Facilitate optional group programming and life-skills development activities.
- Maintain ongoing support and service tracking for each participant throughout their stay.
Rapid Rehousing Services:
- Serve as the point of contact for rapid rehousing clients, assisting with eligibility screening, housing plans, and referrals.
- Provide housing navigation support: locate and secure safe, affordable housing, complete rental applications, and communicate with landlords.
- Coordinate transportation for housing-related appointments and lease signings when necessary.
Post-Exit Aftercare & Follow-Up:
- Deliver up to 12 months of aftercare services for all rapid rehousing clients and First House residents transitioning to permanent housing.
- Conduct follow-up check-ins, help resolve housing stability issues, and reconnect clients with services as needed.
- Monitor participant outcomes using tools such as the MOVERS assessment and Trauma-Informed Practices (TIP) scale.
Collaboration & Advocacy:
- Work collaboratively with the Housing Director, Legacy House staff, and other community partners.
- Advocate for survivors' needs in accessing benefits, healthcare, legal aid, child care, mental health services, and more.
- Participate in community coalitions, trainings, and systems coordination meetings as directed.
Documentation & Compliance:
- Maintain timely, confidential, and accurate case records in Bonterra Apricot or other data systems.
- Contribute to funder reports, internal evaluations, and continuous quality improvement efforts.
Qualifications:
- Associate’s or Bachelor’s degree in Social Work, Human Services, or a related field (equivalent experience will be considered).
- 2+ years of experience in case management, housing navigation, or related supportive services.
- Strong understanding of trauma-informed care and domestic violence
- Familiarity with landlord engagement, tenant rights, rental assistance programs, and community housing resources.
- Excellent interpersonal, organizational, and documentation skills.
- Valid driver’s license and access to reliable transportation required.
