About Job

Job Summary
We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will have experience in case management and a strong background in patient assessment, particularly within hospital settings. This role involves working with diverse populations, including pediatrics and individuals with developmental disabilities, to provide comprehensive care and support. The Social Worker will be responsible for developing treatment plans, coordinating services, and ensuring compliance with HIPAA regulations.

Duties

  • Conduct thorough patient assessments to identify needs and develop individualized care plans.
  • Provide case management services to patients, ensuring they receive appropriate resources and support.
  • Collaborate with healthcare professionals to facilitate outpatient services and coordinate care across disciplines.
  • Maintain accurate documentation of patient interactions and treatment plans in accordance with ICD-10 standards.
  • Engage in research activities to enhance service delivery and improve patient outcomes.
  • Advocate for patients' rights and access to necessary services, particularly for those with developmental disabilities.
  • Participate in multidisciplinary team meetings to discuss patient progress and adjust care plans as needed.

Experience

  • A degree in Social Work from an accredited institution is required.
  • Previous experience in a hospital or outpatient setting is highly desirable.
  • Familiarity with case management practices and patient assessment techniques is essential.
  • Knowledge of HIPAA regulations and compliance standards is necessary for maintaining patient confidentiality.
  • Experience working with pediatric populations or individuals with developmental disabilities is a plus.
  • Strong communication skills, both verbal and written, are critical for effective collaboration with patients and healthcare teams.
