Role: IHS Intake Coordinator (In Home Support)

Location: Grand Junction, CO (On-Site)

ShiftSchedule: Full-Time | Monday-Friday, 8AM-5PM

Pay: $19 - $24/HR | Depending on experience

Requirements: Valid Driver's License, Auto Insurance, and Reliable Transportation (for occasional home visits)

• Homecare and healthcare experience is a plus, but not required

We're looking for an organized, detail-oriented Intake Coordinator to join our team at SummitWest Care in Grand Junction, CO. In this full-time, on-site role, you'll be the first point of contact for individuals seeking in-home support. Help them feel welcomed, informed, and cared for from the very first conversation.

What You'll Do

You'll ensure every client begins their care journey with clarity, confidence, and compassion by:

• Welcoming clients and families as the first point of contact
• Guiding the intake process with care and professionalism
• Scheduling in-home assessments and coordinating next steps
• Gathering essential documentation to ensure compliance and coverage
• Maintaining accurate, up-to-date client records to support seamless care
