Functions and Responsibilities

• Responsible for scheduling or staffing home care staff and ordering equipment and supplies according to the patient's plan of care
• Clinical support for field staff and intake team
• Care coordination with RN case manager
• Understands and adheres to established policies and procedures.
• Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.
• Processes payments and insurance information.
• Attends staff meetings and in-services as scheduled.
• Assists the patient/family in learning appropriate self-care techniques.

Qualifications

• At least one (1) year experience as an Intake Coordinator.
• Acceptance of philosophy and goals of this Home Health agency.
• Ability to exercise initiative and independent judgment with excellent communication skills.
• Completion of a Hospice training program prior to providing care.
