Job Overview:
As a key member of Westminster Place team, you will develop and implement plans to connect intermediate and skilled care residents and their families with our resources and services. Your role will support and enhance their activities within our community.
You will also assess and coordinate the needs of older adults, leveraging the services provided by Presbyterian Living to maximize their engagement and activities both within our community and beyond.
Job Responsibilities:
- Provide short and long term supportive counseling to assist residents in coping with transitions/discharge planning and loss, adjusting to physical, cognitive, and emotional changes, and meet other psychosocial needs as expressed by the resident.
- Act as resident advocate/liaison with their families around issues of health and psychosocial needs.
- Promote effective communication among residents, families and involved staff members.
- Provide counseling and support to families and friends of residents around issues related to the residents’ needs and concerns and maintains open communication with families to keep them informed of resident needs, illness, changes in condition, billing changes and/or service levels within the constraints of HIPAA regulations.
- Facilitate interdisciplinary care plan meetings.
- Crisis management to assist resident and families. Assist resident/families during occurrences.
- Assist residents in obtaining appropriate available services to enhance their ability to function at the highest level of independence possible and to provide support for their physical and psychosocial needs within the constraints of the HIPAA regulations.
- Develop and implement educational and psychologically supportive programming and groups that will benefit residents and/or their family members.
- Inform individuals of their rights and responsibilities as residents of Presbyterian Homes and serve as resident advocate to ensure their rights are honored at all times.
- Compiles, maintains, and updates pertinent personal data, documentation and reports for all residents.
- Provide appropriate social service assessments and documentation in resident records in keeping with state and federal guidelines.
- Responsible to learn and use standard functions within software and know basic operation of computer to enhance efficiency in resident/patient service, assessment and /or clinical documentation. Responsible to use computers and software as required by work environment in order to enhance communications, performance and knowledge.
- Have knowledge and complete the Minimum Data Set (MDS) in order to be Compliant in the medical record.
- Knowledgeable regarding Medicare and Medicaid in order to assist residents/families with these programs.
- Experience in the use of an electronic medical record. In the event of no prior EMR experience, will be required to demonstrate a level of proficiency within the probationary period after orientation and training.
- Represent Westminster Place’s involvement with Voter’s Registration and facilitating resident voting.
- Participate in inter-disciplinary committees related to resident psychosocial needs.
- Coordinate committees related to institutional concerns.
- Provide in-service training of other staff regarding psychosocial needs of residents and the older adult population.
- Participate in professional seminars and workshops.
- Coordinate resident discharge planning.
- Is aware of Presbyterian Living Corporate Compliance policies including those having to do with confidentiality and security of information, and concern for the wellbeing of residents and patients. Demonstrates behaviors and completes work in a manner consistent with these policies.
Education: Master’s degree in social work.

- Counseling skills, communication skills. Capable of independent judgment, initiative, and ingenuity. Ability to become an active participant in the inter disciplinary health care team.
- Have a proactive personality with an emphasis on teamwork and flexibility.
- Must be able to work in an unstructured environment, handle several tasks simultaneously, problem solve and respond quickly and effectively.
- Possess experience facilitating groups.
- Ability to maintain confidentiality.
- Desire and ability to work with older adults. Computer literacy.
