General Position Overview:
The Care Coordinator provides quality Care Management services to homebound, low-income frail older adults and adults with disabilities who have been referred by community partners, family, or self. It is the goal and intention of ONEgeneration’s Care Management to provide the support needed to ensure that their clients (homebound seniors) are safe in their home, properly connected to appropriate resources, and maintain the highest quality of life, reducing hospitalization and long-term placement/institutionalization.
Main Responsibilities:
- Implement all aspects of care management activities, according to agency policy, including in-home assessment; client records; information & referral; case monitoring; counseling, and individualized care plans. Maintain a caseload and ensure all home-visits, assessments, documentation, and care plans are completed in a timely manner.
- Conduct initial and ongoing outreach, information and referral services to incoming clients.
- Make an effort to build rapport and trust with their clients.
- Maintain a caseload which includes follow up visits and telephone calls as required.
- Complete yearly, semi-annual, and quarterly re-assessments to determine eligibility and screening of additional services if needed.
- Provide appropriate crisis intervention to both care management clients and the community at large.
- Oversee one city respite program *program will be assigned.
- Maintain up-to-date documentation for each client served. Update Home Visit and Master List Schedule on a daily basis.
- Comply with all special grant and regulatory guidelines for additional programs outside the City Case Management contract (i.e. fall prevention, emergency preparedness, etc.).
- Actively link and refer clients to appropriate community resources. Stay updated and aware of local resources and new services that would benefit Care Management clients.
- Actively participate as a member of ONEgeneration’s Care Management Team and advocate on behalf of the care management clients.
- Remain up to date on new resources, referrals, etc. by attending regular trainings in specified service area.
- Assist with the ONEgeneration special events as needed.
- Participate in ONEgeneration’s annual Project SHARE (adopt-a-senior) program.
- Maintain ongoing communication with supervisor and attend telephone conference and monthly or bi-monthly in-person meetings.
- Communicates observations and problems to the Lead Care Coordinator or Director of Case Management.
- Communicates all client updates with Lead Care Coordinator and the Director of Case Management.
- And any other job-related request of the Director of Case Management.
Qualifications:
Strong organizational and communication skills. Administrative and record-keeping experience. Ability to work under pressure. BA/BS degree in Social Work, Sociology, Gerontology, Public Health, Psychology or other related degree. Experience working with the frail/elderly. Comfortable in conducting field-based home visits. Must have a working vehicle and driver’s license/insurance. Knowledge of community resources and initiative to seek out additional resources. Skill in verbal and written communication. Bilingual English/Spanish is preferred for this position.
Working Conditions:
Working conditions are normal for an office environment. Must have a working vehicle and driver’s license. This position is not a remote position and will be expected to work on-site.
Job Type:
Full-time, 40 hours per week, nonexempt.
Salary:
$22-23 per hour.
