The VNA Health Group is seeking a Per Diem Medical Social Worker to support the Home Health team in our Englewood team. This position is responsible for providing assessment of social and emotional problems that are, or are expected to be an impediment to the effective treatment of the patient’s medical condition or their rate of recovery.

The VNA Health Group is seeking a Per Diem Medical Social Worker to support the Home Health team in our Englewood team. This position is responsible for providing assessment of social and emotional problems that are, or are expected to be an impediment to the effective treatment of the patient’s medical condition or their rate of recovery.

- Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
- Develops and implements a social work plan of care in partnership with patient, representative (if any) and caregiver(s)
- Collaborates with professional staff on psychosocial problems and appropriate interventions
- Provides patients, caregivers, and/or families with information and education on community resource options and assists in linking with needed resources
- Adheres to the organization’s policy and procedure on documentation
- Participates in organizational wide QAPI program and attends agency sponsored in-service programs
- Adheres to agency infection control and safety policies
- Adheres to the organization’s policy in regards to absenteeism and appearance
- Master’s Degree in Social Work accredited by Council on Social Work Education required with current NJ LSW
- 1+ year of supervised clinical experience in a healthcare setting; community health experience preferred
- Excellent verbal and written communication skills
- Excellent interpersonal skills
- Ability to work with multiethnic and diverse socioeconomic populations
