General Summary/Responsibilities:
Serves as a point of coordination between programs for patients experiencing barriers to mental health care which includes homelessness, food insecurity, financial resources strain, and transportation.
Supports and links patients to various community programs and resources.
Participates as a professional team member in patient care planning along with other hospital and community agency personnel.
Participate in care rounds and other interdisciplinary collaborations within the in-patient and outpatient programs. Frequent contact with community agencies, outside review organizations, and Behavioral Health social workers/counselors
