Starting annual salary $48,000 commensurate with experience

Working Schedule: Sunday-Thursday 1pm-9pm

Program Summary

The Sub-Acute Stabilization program is a short-term, 1-14 days, behavioral health program for youth ages 5 to 18. The interdisciplinary team works together to stabilize, assess, and then discharge back into the community.

Job Summary

This position is a key member of the treatment team and is responsible for the intake process, from gathering referral information, and through the discharge. This person works alongside the clinician and aids in connecting the youth and family to services at discharge.

Key Responsibilities

  • Carries out all required daily responsibilities of central intake functioning, including but not limited to, direct phone contact with clients/consumer, preliminary screening/assessment.
  • Inputs client intake information.
  • Facilitates discharge planning, which may include linkages to community support services.
  • Develops and maintains collaborative relationships with other community agencies and organizations in consultation with supervisor, in order to provide comprehensive and effective treatment services for clients (i.e., schools, courts, hospitals, etc.).
  • Works as a team member with other clinical and administrative staff to respond to client and programmatic needs.
  • Assists families with identifying basic needs; providing or accessing transportation; accessing school, job preparation, employment and housing information; and locating free family recreation activities.
  • Compiles and updates information on resources in the local area for the team.
  • Attends and participates in weekly rounds and group supervision.
  • Provides intensive case management, support services, advocacy, psycho-education and parenting skills training/teaching, and coaching.

Required Experience

EDUCATION

  • Bachelors degree in social work or an equivalent human services field required.

Experience

  • Experience working with children in a healthcare or residential setting preferred.
  • Knowledge of community resources.
  • Bilingual Spanish speaking preferred.
  • Excellent interpersonal, organizational and customer services skills.
  • Flexible and able to multi-task.
  • Proficient in Microsoft Outlook, Word and Excel.
