This Primary Care Medical Center, complete with convenient on-site parking, is expanding its dedicated team and seeking a Behavioral Health Therapist (LCSW or LCPC) to empower individuals with complex chronic and co-occurring conditions.

Your Role in Healing & Hope:

As a Behavioral Health Therapist, you will:

  • Provide impactful individual and group counseling sessions (typically ~6 patients daily).
  • Conduct thorough comprehensive assessments and critical crisis interventions for patients navigating multiple chronic conditions.
  • Thrive in a dynamic, interdisciplinary team environment, collaborating with nurse practitioners, social workers, case managers, medical assistants, and medical providers.
  • Craft individualized care plans and ensure seamless follow-up throughout the care continuum.
  • Empower patients and their families through vital education on how chronic conditions intersect with mental health and wellness.

  • Active LCSW or LCPC license in Maryland is essential.
