About Job

Mission Hospice based in Pasadena, is seeking a Full-Time Social Worker to join our team in the San Fernando Valley Area. The medical social worker provides social service counseling to help coping capacity of each patient and involved caregiver in accordance with the interdisciplinary plan of care and Medicare Conditions of Participation. Provides compassionate care, advocacy, and psychosocial support and education from admission through bereavement.

Master’s in Social Work (MSW) required.

This position is field and area based, which means the counselor would spend majority of their time out of the office meeting families in person or by phone, performing psychosocial and bereavement assessments, or counseling about hospice care.

Description:

  • Provide explanation of the Medicare hospice benefit, as needed.
  • Assesses the psychosocial status of patients and families/caregivers related to the patient’s terminal illness and environment and
  • Communicates findings to the registered nurse and other members of the interdisciplinary group.
  • Provides an assessment in the patient’s identified residence and assistance when this is not safe and another plan is required.
  • Assesses for any special needs related to the culture of the patient and family.
  • Communicate role of family, space, and any special traditions.
  • Maintains clinical records on all patients referred to social work and prepares clinical and progress notes.
  • Educates patients and families on and assists in preparation of advanced directives.
  • Provides information to patients and families/caregivers and community agencies.
  • Serves as liaison between patients and families/caregivers and community agencies.
  • Participates in the development of the individualized plan of care, involving the patient and family.
  • Assists family and patient in planning for funeral arrangements, financial, legal, and healthcare decision responsibilities
  • Serves as liaison between patients and families/caregivers and community agencies.

Shift and Pay

  • Days: Monday to Friday, including monthly weekend rotations
  • Hours: 9 am to 6 pm (8 hour shifts)
  • Pay is hourly, not salary

Job Type: Full-time

Pay: From $35.00 per hour
