About the Role:
We’re hiring a Community Outreach & Intake Coordinator to drive clinic growth by bringing in new patients and helping them stay engaged in treatment.
This is a frontline role that blends outreach, lead follow-up, and team management. You’ll work hands-on with our intake process while also managing two staff members who support your efforts through lead tracking and patient coordination.
Your work will shape not only who walks through the door — but also who stays and succeeds.
What You’ll Do:
- Respond to inbound leads (calls, forms, messages from online ads)
- Work with support staff to ensure new patients attend initial appointments
- Build partnerships with hospitals, shelters, detox centers, and local clinics
- Represent the clinic at events, community meetings, and drop-ins
- Supervise and coordinate two support staff responsible for intake follow-up and patient progress tracking
- Maintain contact with referring partners to ensure strong, ongoing collaboration
- Help new patients feel welcome, informed, and encouraged to complete care

- Have experience in outreach, healthcare, treatment, or admissions
- Are highly organized, self-directed, and people-savvy

- Can talk to anyone — from someone in crisis to a hospital case manager
- Understand the importance of both showing up and following through
Bonus Points If You:
- Have worked in addiction treatment, EMS, behavioral health, or case management
- Have built community or referral relationships in past roles
- Have personal or professional experience in recovery
Compensation & Benefits:
- Annual salary starting around $54,000

Job Type: Full-time
Pay: $54,000.00 - $58,000.00 per year
Schedule:
- Monday to Friday
- Weekends as needed
