Overview
We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will be responsible for providing support and assistance to adults and older adults with various challenges. This role requires a strong understanding of behavioral health, case management, and clinical counseling practices. The Social Worker will work collaboratively with the clinical team to ensure the best outcomes for clients while maintaining accurate documentation.

Responsibilities

• Conduct assessments to identify client needs and develop appropriate treatment plans.
• Provide therapy and counseling services to individuals, and groups experiencing behavioral health issues.
• Maintain accurate clinical documentation and coding in compliance with relevant regulations.
• Collaborate with healthcare teams in hospitals or community settings to coordinate care for clients.
• Participate in team meetings, case reviews, and supervision
• Manage cases effectively, ensuring that clients receive the necessary resources and support.
• Advocate for clients' rights and access to services within the healthcare system.

Skills

• Proficiency in clinical terminology and documentation practices.
• Strong background in behavioral health counseling and clinical counseling techniques.
• Experience with case management processes and strategies.
• Knowledge of coding practices related to clinical documentation.
• Excellent communication skills, both English and Spanish verbal and written, to interact with diverse populations.
• Strong organizational skills to manage multiple cases efficiently.
