The Community Support Specialist (CSS) is an essential part of the Independence Program (IP) team reporting directly to Generations Home Care LLC, as well as the Independence Program Case Manager. Each Veteran Warrior is typically authorized for between 10-20 hours per week with their Community Support Specialist. More than one Veteran Warrior may be assigned to a Community Support Specialist if availability allows for adequate scheduling for the Veterans and their CSS. The male warrior we are hiring for needs a MALE CSS to provide 10-12 hours per week of support services. We provide training, supervision by Neuro Case Management and offer on-going professional enhancement educational webinars.
This is a community-based position working with Veterans who have brain injuries, PTSD, neurological disabilities and other cognitive or physical challenges. While providing functional support to a Veteran Warrior, the CSS is responsible for providing opportunities for increased participation in community activities, increased social capital, improved quality of life and development of meaningful and purposeful activities. This is a PART TIME PRN opportunity and pay is $20.00 and hour plus mileage reimbursement.
Job Responsibilities Based on the established goals of an individualized Care Plan provides Veteran Warriors functional supports to achieve set goals.
Identifies barriers, triggers, or challenges in functional status in order to develop proactive interventions that promote continued independence.
Educates Veteran Warriors on how to develop necessary problem-solving skills and strategies solution driven role.
Provides positive support and be an advocate for the Veteran Warrior.
Provides assistance and guidance with identifying choices and making informed decisions that involve their activities and life.
Assists Veteran Warrior with improving community living and integration skills in a variety of settings such as shopping, church attendance, sporting events, recreation, club participation, classroom support, volunteer/employment support, etc.
Serves as a role model/mentor and maintains good communication/relationships with the Veteran Warrior, their families and referral sources.
Desired Experience Experience working with an individual professionally and/or a loved-one with Traumatic Brain Injury (TBI) and/or Post-Traumatic Stress Disorder a Plus!
Understanding of military culture or Veteran - preferred.
Retired/Semi retired male preferred
Behavioral health experience preferred.
Mentoring, coaching or training experience.
Outstanding organizational skills.
Proven experience working in a collaborative environment.
Caring, compassionate and have a high level of patience.
Must have own vehicle with insurance coverage and be willing to transport veteran
