Position: Home Care Hospice Case Manager

Location: Colorado Springs, CO

Duration: 12 weeks

Hours: Monday through Friday, 8 AM - 5 PM (No weekends)

Pay Rate: Up to $57 per hour

Job Description: Are you an experienced Hospice Case Manager looking for a rewarding contract position? We are seeking a compassionate and skilled Home Care Hospice Case Manager for a 12-week contract in Colorado Springs, CO. Responsibilities:

Manage and coordinate hospice care for patients in their homes.

Develop and implement individualized care plans.

Provide compassionate support and education to patients and their families.

Collaborate with interdisciplinary team members to ensure comprehensive care.

Document patient progress and adjust care plans as needed.

Travel to patients homes using reliable transportation.

Requirements: Minimum of 2 years of experience in hospice care.

Current RN license in good standing.

Reliable transportation for home visits.

Strong clinical skills and ability to manage complex care needs.

Excellent communication and interpersonal skills.

Compassionate and patient-centered approach to care.
