New Vista is currently recruiting for Discharge Planners (BSW required) at Clear Vista, a Behavioral Health Hospital located outside of Cleveland in Lorain, Ohio.

JOB REQUIREMENTS

Education

  • Bachelor’s degree in Mental Health (BSW) and experience with Mental Health population preferred

Certification

  • Handle with Care training and Basic Life Support (BLS) required upon hire or within 30 days of hire

Work Experience

  • One year of experience in a mental/behavioral health and/or Recovery Center preferred

Other Requirements

  • Critical thinking skills, decisive judgment and the ability to work with minimal supervision.
  • Teamwork, flexibility needed to fulfill job responsibilities, adapting to changes in work environment and accepting supervisory feedback.
  • Adherence to all relevant policies, procedures, and guidelines affecting the work environment, as well as maintenance of required competencies and communications skills.

JOB RESPONSIBILITIES

As Discharge Planner, you will:

  • Assume primary responsibility for assigned client caseload.
  • Ensure that all documentation regarding discharge coordination is completed in a clear, concise, professional, and timely manner.
  • Act as liaison between referral sources and clients.
  • Conduct family interview and provide for referral to appropriate resources to include discharge transportation.
  • Complete provision of discharge documentation to next level of care provider.
  • Complete ROI as appropriate for discharge coordination.
  • Provide crisis intervention to clients as needed.
  • Plan aftercare for assigned clients to include case management.
  • Assist with Outcome Measures and Satisfaction Surveys.
  • Perform other duties as assigned.
