Aspirus Health in Wausau, WI is seeking a SOCIAL WORKER ASSISTANT to join our WAUSAU HOSPICE HOMECARE team!
Under general supervision and according to established policies and procedures, assists patients and families in coping with problems resulting from severe or long-term illness, and with difficulties in recovery and rehabilitation. Works with patients and families to obtain psychosocial history, assists patients in resolving environmental difficulties interfering with attainment of maximum benefits from medical care, and conducts routine discharge planning. Directs patients to designated community agencies or resources, as necessary. The Social Worker is a core interdisciplinary team member who, as a professional social worker, provides necessary social work services to patients/families under the direction of a physician.
HOURS: Full Time or 1.0 FTE, 80 hours every pay period. Day shift and will include weekend days
Experience/Qualifications
- Knowledge of social work theories, therapies, and techniques of casework processes, and principles of public welfare at a level normally acquired through completion of a Bachelor’s or Master’s Degree in Social Work or related field. Coursework in Medical Social Work preferred.
- Approximately 2 years of on the job experience necessary in a healthcare setting in order to gain full knowledge of federal, state, and local regulations related to discharge planning and knowledge of health and welfare resources and facilities or 2 years of experience as a Social Worker Assistant.
- A current Wisconsin Social Worker certification or ability to obtain within 6 months of hire.
- Is able to travel to patient/client homes to provide patient/client care.
- Possesses a valid Driver’s License required.
- Possesses automobile liability insurance with minimum $100,000/$300,000 coverage required.
- Provides home based social services under the supervisor of a masters prepared social worker per physician order
- Exhibits strong human relation skills for interacting with patients, patient’s family members, physicians, personnel, community agencies, and other resources.
- Displays analytical skills in order to assess patients’ needs, develop associated discharge planning, and provide sound recommendations and guidance. Displays ability to concentrate and pay close attention to detail when interviewing patients & preparing discharge plans.
- Possesses planning and organizational skills and expert skills in prioritization.
- Understands the nature of social diversity and oppression with respect to race, ethnicity, national origin, color, sex, sexual orientation, gender identity or expression, age, marital status, political belief, religion, immigration status, and mental or physical disability.
- Contributes to the Home Health Quality assessment and/or Performance Improvement Process.
