Job Summary:
The DD Care Coordinator (DD CC) is responsible for overseeing all staffing and related administrative tasks within the Developmental Disabilities (DD) department. The primary goal of this role is to ensure timely and effective staffing coverage, maintain communication with internal departments, and support the program’s operational efficiency by reducing unstaffed hours to below 5%.
Duties:
- The Care Coordinator will be required to do the following:
· Handle all Scheduling/Call outs/Rescheduling of clients and staff effectively and timely. Staff clients’ requests, matching the needs and preferences of the clients with the skills and personalities of the caregiver.
· Monitors missed clock-ins and clock outs and supervise all field operations which includes monitoring employee performance, UpToDate communication with case managers and other relevant stakeholders.
· Report and escalate staffing issues to the DDP/Staffing Case Manager and any needed follow ups with support coordinators, MDs, and social workers.
· Handle and resolve all client and staff complaints/concerns and direct urgent and unresolved issues to the QA coordinator to ensure client retention.
· Carry out performance evaluation on all staff (DSPs, LPNs, RNs) twice a year.
· Work with the HR coordinator on employee hiring and orientation procedures to company’s standard
· Work with the DDP/director to manage progress notes, goal tracking and aggregate, fire drills and other pertinent administrative tasks.
· Track and manage caregiver’s availability and overtime status.
· Work with the intake officer and DDP on all referrals (Skilled and Unskilled).
· Staffing – ensuring the appropriate amount of staff is available for each work shift.
· Amending the staff schedule as requested and/or necessary to provide quality patient care.
· Finding coverage for all staff and nursing time-off requests and callouts before after-hours.
· Providing administrative support to meet deadlines, as needed.

QUALIFICATIONS:
A track record of experience in the Healthcare field. Positive attitude with compassion and patience. High School diploma minimum, college degree is a plus. Prior experience with the developmental disability services is a bonus.
Job Type: Full-time
Pay: $18.00 - $19.00 per hour
