Job Overview
The Service Coordinator plays a vital role in ensuring the effective delivery of services to individuals with disabilities. This position involves coordinating programs, managing relationships, and overseeing administrative functions to enhance the quality of life for clients. The ideal candidate will possess a strong background in program development and management, coupled with a passion for supporting individuals with diverse needs. Must be currently working in the CHC/LTSS industry. Duties
- Connect consumers that are in need of our services (Home healthcare, Housing, utility assistance, prevention of eviction, etc.)
- Manage relationships with clients, families, and service providers to ensure effective communication and coordination of services.
Skills
- Strong expertise in program development and management.
- Experience working with individuals who have disabilities/elderly
- Excellent relationship management skills to build rapport with clients and stakeholders
- Strong administrative capabilities, including documentation and reporting skills.
- Background in CHC/LTSS industry
