Job Overview
The Service Coordinator plays a vital role in ensuring the effective delivery of services to individuals with disabilities. Must be currently working in the CHC/LTSS industry. Duties

• Connect consumers that are in need of our services (Home healthcare, Housing, utility assistance, prevention of eviction, etc.)
• Manage relationships with clients, families, and service providers to ensure effective communication and coordination of services.

Skills

• Strong expertise in program development and management.
• Experience working with individuals who have disabilities/elderly
• Excellent relationship management skills to build rapport with clients and stakeholders
• Strong administrative capabilities, including documentation and reporting skills.
• Background in CHC/LTSS industry
