Summary:

PerformCare provides access to the NJ Children’s System of Care. The Behavioral Health Care Manager is responsible for the daily functions of the clinical department including triage of calls, care coordination, and utilization management. The Care Manager facilitates access to behavioral health, substance use, and intellectual/developmental disabilities therapeutic services, ensuring that every youth receives the right services at the right time as part of an integrated service plan. The Manager makes the timely intensity of services determinations based on the urgency of need, including the management of emergencies, which includes access to mobile response services.

Responsibilities:

  • Makes timely intensity of services determinations or refers to higher-level review when indicated
  • Provides linkage to community services as needed
  • Adheres to customer service level agreements and expected productivity measures
  • Adheres to documentation requirements
  • Identifies a wide range of community resources to the caller
  • Demonstrates respectful communication and excellent customer service
  • Adheres to recognized ethical standards
  • Assists with projects as assigned

Education/Experience:

  • Graduate degree from an accredited educational program in Social Work, Clinical/Counseling Psychology;
  • Active NJ clinical licensure as a Licensed Clinical Social Worker (LCSW), a Licensed Marriage and Family Therapist (LMFT), a Licensed Professional Counselor (LPC), or a Licensed Psychologist
  • Minimum of two (2) years experience in children’s mental health, DCPP, juvenile justice, and/or a related public sector human services or behavioral healthcare field, providing community-based services to children, youth, and young adults, and their families/caregivers
  • Background and experience in family systems, community systems and resources, case management, child and family counseling/therapy, child protection, and/or child development
  • Clinical and cultural competency with the training and experience necessary to manage complex cases in the community across child-serving systems
  • Ability to screen and assess crisis and emergency calls and assess the caller’s degree of acuity/severity and clinical necessity for treatment based on DCF/CSOC-approved criteria
  • Must be computer savvy
  • Ability to multi-task
  • Demonstrate knowledge of a variety of approaches to intervention and support
  • Bilingual (Spanish) is helpful
